Coleman Medical Associates was established in 1995 to provide the residents of Coleman and surrounding counties with a range of readily accessible, high quality primary care services focused on keeping the entire family healthy.

In recognition of this commitment, Coleman Medical Associates became a certified Patient Centered Medical Home and embraced a model of healthcare delivery that allows our staff to work with you to assess your health status, address specific problems and concerns and coordinate the specialty care provided by other healthcare professionals.   

We serve as your advocate in all health-related matters, including the appropriate use of specialists, health services, and community resources.

Clinic Services

Management and Treatment of Chronic Medical Conditions

Effective management and treatment of various chronic medical conditions is important to maintaining a healthy, active lifestyle, and the prevention and management of complications.  Some of the chronic medical conditions managed by your healthcare team at Coleman Family Medicine include:

Diagnostic and Screening Tests

Coleman Medical Associates offers a wide range of diagnostic tests to assist with the development of your wellness or treatment plan. Services may be provided in the clinic or Coleman County Medical Center.

Chronic Care Management

If you have chronic health care problems, there is a new “Coach” in town who may be able to help you improve your quality of life.  Michelle Burdick, RN, is a certified Clinical Health Coach and helps Medicare patients enrolled in the Chronic Care Management program at Coleman Medical Associates to play a more active and effective role in the management of their health.  The Chronic Care Management program was established to help Medicare patients with two more chronic conditions – such as diabetes, arthritis, high blood pressure, heart disease or stroke – play a more active and effective role in the management of their health. 

As a Health Care Coach, Burdick meets with each patient enrolled in the program to talk about their health care needs, concerns and goals.  Together, Burdick, the patient and their physician develop an individualized plan to accomplish these goals, and monitor progress over time, making adjustments to the plan as needed.  It is her job to work with patients to give them the tools they need to succeed.

A registered nurse, Burdick received her Clinical Health Coach training and certification from the Iowa Chronic Care Consortium, a voluntary group of public and private organizations established to help patients more effectively manage the most common chronic diseases.

Medicare patients with  chronic healthcare conditions may join the program by calling Michelle at 325/625-3533 to schedule an Annual Wellness Visit with their physician and discuss enrollment.  Michelle is available to answer
any questions patients may have about the program, including whether or not an individual qualifies for participation.  Coaching services provided by Michelle are free-of-charge to eligible Medicare patients. 

Medicare Wellness Visits

If you are covered by Medicare, you are eligible for a free Annual Medicare Wellness Visit to talk with your provider about ways to improve your health and prevent disease.  The Annual Wellness Visit is not the same as an annual physical exam, but it does provide patients with the same opportunity to talk with their provider about health concerns and to develop a plan in partnership with those providers to focus on health, rather than illness. 

Some of the services covered during an Annual Wellness Visit include:  routine measurements such as weight, height and blood pressure; review of your individual and family medical history, review of your current medications, discussion of the care you receive from other providers, and review of your functional ability and level of safety.  Based on this review, you and your provider will develop a health promotion and disease prevention plan based on your specific needs.

After your initial wellness visit, you are eligible for a follow-up or subsequent wellness visit every 12 months.  These visits are not subject to copayments to deductibles, so there is no cost to the individual.  Call Coleman Medical Associates at 325/625-3533 to schedule your Annual Wellness Visit.

Coleman Medical Associates is housed on the second floor of Coleman County Medical Center at 310 S. Pecos Street, Coleman, Texas 76834. Please enter through the main door of Coleman County Medical Center, take the elevator to the second floor and turn left. Please feel free to ask any hospital or clinic staff member if you need additional assistance.

Coleman County Medical Center and Coleman Medical Associates serve all patients regardless of ability to pay. 

Discounts for uninsured patients seeking medically necessary care provided by the hospital and clinic are offered based on the size of the patient’s immediate family, annual household income, and a sliding scale comparing household income with the hospital and clinic’s charity care income guidelines.  

Uninsured patients may apply for a discount in the hospital business office, or clinic front desk.

Our doctors contract with most major health insurance companies and participate in the plans these companies offer. A partial list of insurance companies with which we have contracts can be found below. Please feel free to call us if you have any questions.

If you have medical insurance, please provide that information to our staff when you make your appointment. We will verify your eligibility and file a claim on your behalf.

Providing your insurance information in advance will help speed you through check in at the time of your appointment.

Copays, coinsurance and deductibles are due at the time of service. Not all services, even those most people consider routine, will be covered by every insurance plan. If you have questions about specific benefits, please contact your insurance carrier. Services not covered by your insurance plan are the patient’s responsibility.

Darron T. Atwood, MD

Dr. Atwood specializes in family medicine and emergency medicine. He has served the residents of Coleman County for almost 15 years. He completed his residency at the John Peter Smith Hospital Family Practice Internship and Residency program, Ft. Worth, and earned his medical degree from St. George’s University School of Medicine, Grenada, West Indies. Dr. Atwood received his undergraduate degree from the University of Texas, Arlington. He is board certified by the American Board of Family Medicine.

J. Paul Reynolds, MD

Dr. Reynolds has been a member of the Medical Staff at CCMC for almost 25 years. He completed his residency training at John Peter Smith Hospital Family Practice Internship and Residency program and earned his medical degree from Texas Tech University of Health Sciences Center, Lubbock. Dr. Reynolds completed his undergraduate work at Baylor University, Waco. He is board certified by the American Board of Family Medicine.

David Longley, FNP-C

David is a certified nurse practitioner who moved to the U.S. from England to study nursing. He received his LVN and RN at Ranger College in Brownwood, Texas, and Excelsior College, New York City, New York, respectively. He received his Bachelor of Science in nursing, Master of Science in nursing and Family Nurse Practitioner certification from Abilene Christian University, Abilene, Texas.
